🛡️ What Condoms Actually Protect Against — and What They Don't

Condoms are one of the most widely used and well-known tools in sexual health. But there's a significant gap between what most people believe condoms protect against and what the evidence actually shows. That gap has real consequences — because overestimating condom protection leads to undertesting, and undertesting leads to undetected infections.

What condoms protect against effectively

When used correctly and consistently, condoms are highly effective at reducing the risk of several STIs.

HIV — Condoms significantly reduce the risk of HIV transmission. Studies suggest that consistent, correct condom use reduces the risk of HIV transmission by approximately 80–85%. Combined with other prevention tools like PrEP, the risk reduction is even greater.

Chlamydia and Gonorrhea — Both of these bacterial infections are transmitted primarily through genital fluids. Condoms create a physical barrier that prevents fluid exchange, making them highly effective at reducing transmission when used correctly.

Trichomoniasis — Similarly transmitted through genital contact and fluid exchange, trichomoniasis is well-protected against by consistent condom use.

Hepatitis B — Transmitted through blood and sexual fluids, Hepatitis B transmission is significantly reduced by condom use. Vaccination provides additional protection.

For infections that are primarily transmitted through fluid exchange during penetrative sex, condoms work well — provided they're used correctly, consistently, and throughout the entire encounter, not just at certain moments.

Where condoms fall short

Herpes (HSV-1 and HSV-2) — Herpes spreads through skin-to-skin contact, not just through fluids. Because condoms don't cover all the skin in the genital area, they provide incomplete protection against herpes. An infected area not covered by a condom can still transmit the virus. Research suggests condoms reduce herpes transmission risk by around 30% — meaningful, but far from complete.

HPV (Human Papillomavirus) — HPV is similarly transmitted through skin-to-skin contact. Condoms reduce HPV transmission risk, but because the virus can be present on skin that isn't covered, they don't eliminate it. Mpox — Mpox spreads through direct skin-to-skin contact and can affect areas not covered by a condom. Condom use reduces but does not eliminate transmission risk.

Pubic Lice — Pubic lice spread through close physical contact and can be present in areas not covered by a condom. Condoms offer no meaningful protection against pubic lice.

The correct use problem

Even for the infections where condoms are most effective, the protection depends entirely on correct and consistent use. Studies on condom effectiveness are typically conducted under conditions of perfect or near-perfect use — which doesn't always reflect real-world behaviour.

Common errors that reduce condom effectiveness include not using a condom from the beginning of sexual contact, using the wrong size, using oil-based lubricants with latex condoms (which degrade the latex), storing condoms in conditions that damage them (wallet, hot car), and not leaving space at the tip.

Real-world effectiveness is lower than clinical trial effectiveness — which is worth keeping in mind when making decisions based on condom use alone.

The takeaway isn't that condoms aren't worth using. They are. Consistent condom use provides meaningful protection against a significant range of STIs and remains one of the most important tools in sexual health.
